Prepare Your Pet for a Smooth Diagnostic or Surgical Visit

Except in emergency situations, the doctors and/or staff of Kyle Animal Hospital will present a specific treatment proposal and estimate of costs prior to procedures being performed. The medical plan and estimate of costs is a description of the procedures deemed necessary and appropriate at this time by the doctors at Kyle Animal Hospital for your pet. Prior to allowing any procedures on your pet, there are some things we would like you to know:

  • Emergency conditions may arise that necessitate further treatment. In the event such conditions arise, and you cannot be contacted, by this agreement, you authorize and agree to pay for such emergency treatment as necessary for the health of your pet.
  • You should know that no personnel of Kyle Animal Hospital will be present overnight, nor continually during the day on weekends. Several 24-hour hospitals/critical care facilities are in the area, and you may transfer your pet to them for overnight care if you prefer. This will be especially important for neurological or respiratory cases, or other cases deemed necessary by the attending veterinarian.
  • The client is responsible for all fees incurred during this visit, including those not specified in the estimate and associated with complications from medication, treatment, vaccination, diagnostic procedures, etc.
  • If fleas or ticks are found on your pet, they will be treated at your additional expense.
  • There is a risk of complications, including death, for any procedure that may be performed, including basic vaccinations, lab tests, etc. These risks are greater with procedures involving surgery, anesthesia, etc. Please ensure that the nature and risks of any procedure(s), have been explained to you or see that they are explained to you, and any questions that you may have answered to your satisfaction, before you will allow treatment. Please understand that there will be no guarantees as to the results of any procedure, medication, therapy, preventive therapy, or test. You do have the choice to obtain additional information regarding opinions for preventive care, therapeutics, diagnostics, or any other procedure for your pet.
  • In order to expedite care of your pet if you are dropping him/her off prior to an examination by a doctor, the technicians and/or receptionists of Kyle Animal Hospital may prepare a preliminary medical plan for your pet. You will have the option of approving this plan at the time you drop your pet off. This plan is based on generally accepted guidelines established by the doctors of Kyle Animal Hospital in order to establish a minimum data base of information. The recommended procedures may vary considerably after a doctor’s examination, and you will be contacted for approval if the cost for the recommended procedures varies by more than 10%. Should you not approve this plan in advance, you will still be responsible for the office call/examination fee prior to your approval of other procedures.
  • Special note regarding declaw procedures: Declaw procedures entail more risk of complications than most other elective surgeries. This is an accepted risk, and clients assume full responsibility, financial and otherwise, for these complications. Some can be long-term and expensive. If you desire more information than can be covered on a standard release form, please ask and be certain you are fully informed to your satisfaction prior to surgery. If you are unsure, there are other options which our doctors and staff will be happy to explain.
  • The presented proposal is a good-faith diagnostic and treatment plan for your pet. Further procedures may be found to be necessary, pursuant to this initial plan. Should such subsequent procedures be recommended, they will only be performed after you have been contacted and have authorized such procedures, except in the event of an emergency. Your signature authorizes this written consent to be applied to such procedures after my verbal, written, or electronic approval of such procedures.
  • Your signature indicates that you have reviewed and agree to the terms of the estimate, plan, and information/authorization form, but does not make you responsible for the charges unless performed on your pet. Payment is due at the time of service, unless other arrangements have been made in advance. If payment is an issue for you, please discuss this in advance of services being rendered. Theft of services is a crime and will be prosecuted. If further conditions are uncovered in the due course of the treatments and diagnostics described above, that exceed by 10% the cost of the estimate, they will be treated by the doctors and staff of Kyle Animal Hospital only after we have attempted to contact you, or it is an emergency treatment as noted above.

If you desire more time to investigate the recommended vaccine and wellness protocol, either on your own or with our doctors, please do not hesitate to inform us. We want you to be comfortable with all procedures we prescribe.
