Share Your Pet’s Wellness Needs With Our Team

The doctors and/or staff of Kyle Animal Hospital will present a specific wellness plan proposal and estimate of costs prior to procedures being performed. Prior to allowing any procedures on your pet, there are some things we would like you to know:

  • Vaccines are not without risk. Some vaccine-associated adverse events can be life-threatening and are impossible to predict.
  • Vaccine-associated adverse events, such as reactions, hives, vomiting, etc., are something we take very seriously. If your pet has ever had an adverse reaction to a vaccine, please bring it to our attention and ensure that it is fully understood by our staff prior to authorizing any procedures. You cannot overemphasize this too much. Do not authorize any treatment or procedures until you have had the opportunity to discuss the recommended vaccine protocol and risks versus benefits of vaccination for your pet, until you have ensured that we understand your pet’s lifestyle, and any questions you may have answered to your satisfaction. Should your pet’s lifestyle change (indoor/outdoor, boarding, grooming, hunting, exposure to other animals, rattlesnakes, etc.) you should contact Kyle Animal Hospital for a review of your pet’s vaccine protocol.. No guarantees of any kind are made as to the outcome of this or any other treatment or procedure.
  • The client is responsible for all fees incurred during this visit, including those not specified in the estimate, as well as treatment for vaccine-associated adverse events.
  • If fleas or ticks are found on your pet, they will be treated at your additional expense.
  • There is a risk of complications, including death, for any procedure that may be performed, including basic vaccinations, lab tests, etc. Please ensure that the nature and risks of any procedure(s), have been explained to you or see that they are explained to you, and any questions that you may have be answered to your satisfaction, before you will allow treatment. Please understand that there will be no guarantees as to the results of any procedure, medication, therapy, preventive therapy, or test. You do have the choice to obtain additional information regarding opinions for preventive care, therapeutics, diagnostics, or any other procedure for your pet.
  • In order to expedite care of your pet if you are dropping him/her off prior to an examination by a doctor, the technicians and/or receptionists of Kyle Animal Hospital may prepare a preliminary wellness plan for your pet. You will have the option of approving this plan at the time you drop your pet off. This plan is based on generally accepted wellness protocols established by the doctors of Kyle Animal Hospital, and will be reviewed by the doctor(s). However, the recommended treatments may vary considerably after a doctor’s examination, and you will be contacted for approval if the cost for the recommended procedures varies by more than 10%. Should you not approve this estimate in advance, you will still be liable for the office call/examination fee until you approve other procedures.
  • The presented proposal is a good-faith diagnostic and treatment plan for your pet. Further procedures may be found to be or become necessary, pursuant to this initial plan. Should such subsequent procedures be recommended, they will only be performed after you have been contacted and have authorized such procedures, except in the event of an emergency. Your signature authorizes this written consent to be applied to such procedures after my verbal, written, or electronic approval of such procedures.
  • This medical plan recommendation is made as a good-faith effort to address issues that may affect your pet’s health and the health of humans it may contact, but the doctors of Kyle Animal Hospital cannot foresee every possible risk scenario.
  • Your signature indicates that you have reviewed and agree to the terms of this authorization, but does not make you responsible for the charges listed unless performed upon your pet. Payment is due at the time of service, unless other arrangements have been made in advance. If payment is an issue for you, please discuss this in advance of services being rendered. Theft of services is a crime and will be prosecuted. If further conditions are uncovered in the due course of the treatments and diagnostics described above, that exceed by 10% the cost of this estimate, they will be treated by the doctors and staff of Kyle Animal Hospital only after we have attempted to contact you, or it is an emergency treatment as noted above.

If you desire more time to investigate the recommended vaccine and wellness protocol, either on your own or with our doctors, please do not hesitate to inform us. We want you to be comfortable with all procedures we prescribe.
